Unable to Submit a Share For Reimbursement

Your dentist is affiliated with Bento. Affiliated providers submit claims directly to Bento so that you do not need to worry. Please reach out to your Bento Concierge at smile@bento.net if you need further assistance. 

Most affiliated dentist offices will your share to Bento automatically if they are aware of your dental membership. However,  in the event that yours provided you with other information you may need to contact your dentist, provide them with your member ID card or plan documents which will remind them on the ways to submit claims for payment.
