Please reach out to your dentist and provide them with your dental membership details before submitting your sharing request to Bento.

Most dentist offices will submit requests on your behalf to your dental membership if they are aware of your membership.

Please call your dentists, provide them with your membership ID, our support number (800-734-8484), and ask them if they are able to submit the request to Bento.

When they call us, we'll provide them with instructions for submitting a sharing request and confirm your membership details.

If your dentist is in network with Bento then they will need to submit the claim. Email smile@bento.net if you need further assistance. 

If you submit a sharing request without notifying your dental office that your have a dental membership, Bento may not be able to process the sharing.
